After submitting your form, you will receive an email from Carepatron, our clinic scheduler. Emails must be verified otherwise we may not receive your form. If you do not receive an email from no-reply@carepatron.com within 5 minutes, please check your spam or junk folders.
Step #3: Scheduling your appointment:
After submitting your form and verifying your email, we’ll review it and contact you within 1–2 business days with a scheduling link by email and/or text.

Changes can be made on your client portal or on the appointment emails you receive. Please do not schedule a second appointment above unless you have missed or completed your first appointment.
Please note: Appointments needing to be modified by rescheduling or canceling less than 72 hours or 3 days before appointment start time incur an automatic late modification/late cancelation fee per clinic policy.
