Isaac Diaz, MPH, RDN, LDN, NASM-CPT, CHES is a Registered Dietitian Nutritionist licensed in Alabama, Alaska, *Arizona, Arkansas, *California, *Colorado, Connecticut, Delaware, Florida, Georgia, Idaho, Illinois, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maine, Maryland, Massachusetts, *Michigan, Minnesota, Montana, Nebraska, Nevada, New Hampshire, *New Jersey, New Mexico, New York, North Carolina, Ohio, Oregon, Pennsylvania, Rhode Island, South Carolina, South Dakota, Tennessee, Texas, Utah, Vermont, *Virginia, West Virginia, Wisconsin, and Wyoming. States with the (*) asterisk above do not require licensure. If your state of residence is not listed above, we are unable to provide services at this time. However, we expect to be licensed in every state by mid April 2026.

Do you offer weight loss and fitness support for clients not taking GLP-1 medications?

Yes, we work with clients who are not taking GLP-1 medications. We also support clients focused solely on weight loss, as well as those seeking additional fitness and physical activity guidance.

Do you offer meal and fitness plans?

Meal plans are not included and are generally discouraged, as they do not promote long-term, sustainable eating habits and require significant time outside of sessions to create. Insurance does not cover the cost of meal plans. Instead, we provide personalized meal ideas, strategies, and education to help you develop lasting healthy eating habits.

Insurance also does not cover fitness or exercise plans. However, we can provide evidence-based resources and guidance to support your individual fitness and physical activity goals.

What should I expect during our appointment?

Sessions are 55 minutes and focus on your individual health and nutrition goals. You can expect a personalized discussion of your goals along with evidence-based nutrition education tailored to your needs (e.g., weight management, physical activity, and GLP-1 medication support).

We recommend joining your session on a computer, laptop, or tablet, as screen sharing is used for most of the appointment.

Do you offer in-person services?

At this time, we only offer telehealth video services.

Do you offer Spanish speaking services?

Yes, spanish-speaking services are available.

How do I modify an existing appointment?

Changes or cancellations can be made on your Carepatron client portal or on the appointment reminder emails you receive. Appointments needing to be rescheduled or canceled less than 72 hours or 3 days before appointment start time incur an automatic late modification/late cancelation fee. This is explained in detail in our policy form.

Are you in-network with Medicare, Medicaid or any government health insurance plan, Cigna, or similar?

No, we are out-of-network with all state and federal government-funded insurance plans such as Medicare, Medicaid, any plan that is a “Medicare Advantage (HMO or PPO) plan,” even if it is provided by our in network partners. Clients with these plans may privately pay via self-pay or use a pre-existing HSA/FSA card for services.

If you have a Cigna PPO plan or a similar private insurance plan with another company, you may be eligible for partial reimbursement for out-of-network services, depending on your specific benefits. 

How Out-of-Network Reimbursement Works

1. Pay for Your Session
You pay the full 55-minute session fee of $180 USD using a credit/debit card or HSA/FSA card.

2. Receive a Superbill
After your first visit, we will provide you with a superbill, which documents the services provided. You can submit this to your insurance company to request out-of-network reimbursement.

3. Reimbursement (If Applicable)
If your plan includes out-of-network benefits, your insurance company may reimburse a percentage of the session fee. Reimbursement is typically sent to the original payment method or directly to you—please confirm these details with your insurance provider before booking an appointment.

Do you service U.S. Territories and international clients?

For legal and licensing reasons, we can only work with clients who are physically located in the United States including Washington D.C.. If you are traveling outside the U.S., including U.S. territories (Puerto Rico, Guam, U.S. Virgin Islands, American Samoa, or Northern Mariana Islands), please avoid scheduling sessions during that time.

Where can I find more information about your business?

We are registered with the State of California as an incorporated business, have a business license with the City of Ontario, and are registered with the National Plan and Provider Enumeration System (which issues a National Provider Identifier). This identifier is required for HIPAA-covered health care providers and organizations in the United States.
